NEW Procedure for Physicals

You must complete the RankOne enrollment. This is the new way Creekview Athletics is receiving physicals. Below are the documents on how to set up your account and complete the form.

Step 1

Get a physical
 (doctor's portion)
 

Step 2

Enroll an online account through RankOne if you have not already. 

Step 3

- Follow the steps to entering your athlete's information, uploading your athlete's most recent physical and filling out the 'Cherokee County portion' for the physical
